The Office of the City Clerk is made up of two distinct offices: The
Office of the City Chamberlain and the Office of Vital Statistics.
Office of the City Chamberlain
– Main
switchboard for City Offices, Information services for County and
States Offices, Dog Licenses, Dog Redemption, Handicap Permits, Notary
Public, Hunting/Fishing Licenses, Bingo/Games of Chance/Bell Jar
Licenses, Amusement Permits, Docking Permits, Coin Machine Operator
Permits, Auction Permits, Going Out of Business Sale Permits, Pawn
Broker Permits, Taxi Driver Licenses, Taxi Cab Permits,
Hawking/Peddling Permits, Reservations for Common Council Chambers,
Schedule various meetings and legal notices, File Local Laws with State
Secretary, minutes of all meetings scheduled by the Common
Council
and Mayor, Sales of Documents (zoning book/map, draft plans, etc.),
Freedom of Information Requests, Notices of Claims, Charter and Code
updates, Liquor License requests, Police Department Complaints, Taxi
Cab Driver Complaints, City Official Appointments, Marriage
Affidavits/Licenses, Marriage Transcripts, and Marriage
Officer.
Office of Vital Statistics: Birth
Certificates, Death
Certificates, Birth Amendments, Death Amendments, New Births,
Burial Permits for Funeral Homes, Paternity Acknowledgments, Genealogical
Searches for Birth, Death, and Marriage Records.

8:30 a.m. to 3:45 p.m.
Monday through Friday
$40.00 fee; both parties must be present at same time

Dog
Licenses
Dog licenses done by mail must be accompanied by proof of rabies (1yr or 3yr vaccine only on stationery from vet) and a self-addressed stamped envelope for return receipt must also be
enclosed.
