Birth, Marriage, Death Certificates

The Office of the City Chamberlain provides administrative services to the citizens of the City of Poughkeepsie. 

In order to process your request for birth/marriage/death certificates, please visit our Forms & Applications page. Print the application of the document you are requesting and return the completed form to the Chamberlain’s office;

Marriage Licenses are processed by appointment only. Please call at 845-451-4225 or email our Marriage Officer to schedule an appointment available Monday through Friday from 8:30 am to 3:45 pm.

If you don't have access to email or fax, please place your completed application in an envelope addressed to Vital Records along with a copy of your driver's license, a money order for $10 (per requested copy) and a self-addressed stamped envelope in the drop box near the Police Station.
