Birth, Marriage, Death Certificates

In order to process your request for birth/marriage/death certificate, please visit our Permits & Forms page. Print the application of the document that you need and fill it out. 

Your request may be emailed to [email protected] or faxed to (845) 451-4239.

Marriage Licenses will be processed by appointment only. Please call or email to schedule an appointment.


If you don’t have access to email or fax, please place your application in an envelope addressed to Vital Records along with a copy of your driver’s license, a money order for $10 per copy and a self-addressed stamped envelope in the drop box near the Police Station.  We cannot offer express service during this time.
